This application can be obtained from a certified insurance agent. A medical underwriter at the insurance company would review the application and determine if the applicant is eligible for the policy. The insurance company will not cover any preexisting conditions for up to 1 year after the effective date of the policy. However, the applicant is eligible for prior creditable coverage towards any waiting period for preexisting conditions he/she has been under an individual or group insurance previously without a break of coverage of more than 62 days.
